Build your group’s Superior Wellness Bundle
It’s easy to enroll your group in a Superior Wellness Bundle through SDC! Simply follow these steps:
- Choose an SDC dental plan(s)
Groups must first enroll in an SDC dental plan to be eligible for an exclusive Superior Vision plan through SDC.
- Add your choice of Superior Vision plan
Complete your SDC bundle by selecting one Superior Vision plan to add to your SDC dental plan(s). Only one Superior Vision plan may be selected per group.
- Select your bundled option:
Tied to Dental: Superior Vision plan enrollment mirrors SDC dental plan enrollment exactly—employees and their dependents who enroll in dental are automatically enrolled in the vision plan.
Employer-Paid: The employer pays any portion of the employee premium and 0% of the dependent premium. Employees may enroll in dental only, vision only, or both. Dependents may enroll in each plan in which the employee enrolls.
Voluntary: The employer pays 0% of the employee premium and 0% of the dependent premium. Employees may enroll in dental only, vision only, or both. Dependents may enroll in each plan in which the employee enrolls.
- Indicate your billing preference
Choose to have your group’s dental and vision rates listed separately on the bill or have them bundled together into one rate.

SDC dental plan ID cards will be mailed directly to members. Superior Vision plan ID cards will be mailed separately—members will receive their Superior Vision ID card approximately two weeks after SDC dental ID cards are mailed.
